Is the programme right for me?
This programme is not for everyone. It is important to think about whether this is the right type of support for you so we would encourage you to read more about the programme.
The NHS Type 2 Diabetes Path to Remission Programme (T2DR), formerly NHS Low Calorie Diet Programme (LCD), is group-based and delievered remotelyover video calls. You can find out what to expect over the 12-month programme below.
Before the programme starts
Select a time and date for your telephone-based Initial Assessment
Once we receive your referral from your GP with all the correct information, we will contact you via email within 2 weeks, to ask you to book a suitable time and date for a telephone-based Initial Assessment (IA) with a Health Coach.
Call with your Health Coach
A Health Coach will call you at your chosen date and time to confirm your eligibility, discuss the programme, and agree upon a start date.
Order your starter pack
After your IA, we will provide you with a unique order code for a starter pack to sample each flavour of the total diet replacement products (Strawberry, Banana, Chocolate, Vanilla, Chicken Soup, Vegetable Soup) so you can decide what you would like to order for the full pack.
Prepare to get started
Next you’ll get set up for your start date. There may be a wait for this but you will be informed of this when speaking with your Health Coach:

During the programme
Video call consultation with your Health Coach
On your programme start date, you’ll attend your pre-booked video call consultation with your Health Coach to run through any last questions you may have, record your baseline biometrics (weight, blood glucose and blood pressure), set your personalised goals and discuss what you hope to achieve over the course of the next 12 months.
Attend your first online group session
As your programme is group-based and delivered remotely, you’ll have your first live group session to meet your fellow group members within the week of your start date. This will be through the Liva app or through your web browser.
Attend follow up online group sessions
Whilst you’re settling into the programme, these live group sessions will be weekly, and after the initial few weeks, live group sessions are conducted less often. Committing to attending the majority of webinars will be important for your programme participation.
Receive regular 1-1 text and video message check-ins
You’ll receive regular 1-1 text and video message check-ins from your Health Coach through the Liva app in between group sessions to make sure you’re feeling supported, as well as a one-to-one video call to help you transition into each new phase of the programme.
Connect with group members
You’ll also be able to stay connected with your group members throughout the programme in a closed group chat, and be able to track all of your goals in-app.
Track your measurements throughout your programme
We’ll also need you to continually track your measurements throughout your programme, for safe participation. We’ll keep your doctor in the loop if any of your biometrics could do with a follow-up from them.

After the programme
Continue your health journey
You will continue to be invited to attend your yearly diabetes reviews with your GP and/or diabetes team, and we will signpost you to any relevant healthy lifestyle services in your area to continue your health and wellbeing journey.

I’d like to join the programme
We currently provide the programme in Lancashire & South Cumbria.
If you live within this area, contact your GP practice to discuss your referral into the programme. They will need to complete a review of your clinical status, complete a referral form and share it with us.
Once we receive your referral with all the correct information, we will contact you via email within 2 weeks.

If you are no longer interested in joining the NHS Type 2 Diabetes Path to Remission Programme, you do not need to do anything.
Liva will not contact you, but you may wish to get in touch with your GP to discuss other options for support.
